During FY 2021-22, CARE India reached out to 84.2 million people directly through 88 programs across 21 states.

Objective: The main goal of the proposed project is to collaborate with Dell Technologies as a development partner agency to establish and demonstrate a Centre of Excellence NCD Model Districts where there is more uptake of Continuum of Care (CoC).

The project’s goal is to develop and use digital training tools to improve capacity building in NCD applications across the continuum of care.

Care India will closely collaborate with the chosen state’s health department to establish an NCD Model District and determine the best ways to use the Digital Life Care NCD application to promote the use of continuum of care.

Geography: CARE India can work with communities across the country in different states and have been working with communities, governments, and private sector for improved health outcomes, livelihoods and educational advancement. However, the focus should be primarily where CARE has long standing relationship with government stakeholders during the recent past which can help in leveraging the benefits of the partnerships for the product development process. Furthermore, the district/s chosen for the purpose must have good network connections and enough mobile phones and tablets with users to facilitate the adoption of the Continuum of Care in NCD. Also, there should be no shortage of human resources, such as ASHAs, ASHA Facilitators, CHO/ANMs, and MOs, who have a direct impact on the proposed project’s success. The proposed model district/s will be chosen collaboratively based on DELL preferences and CARE’s existing relationships with state/district officials. CARE will be in charge of obtaining the necessary formal communication from the selected district administration in order to obtain cooperation during the project period.

JOB SUMMARY
The CoC Coordinator will be responsible for rolling out pilot project and support Govt. to strengthen the NCD based Continuum of Care by using digital platforms through ASHA,ANM,CHOs,MOs and Specialists in assigned block. S/he will be responsible for timely execution of rollout plan at block level. S/he will provide insure the day-to-day program activities under the guidance of District Project Manager.

ROLES AND RESPONSIBILITIES
The primary duties and responsibilities of the CoC Coordinators will include the following but not be restricted to:

  • Briefing of Govt. officials/BMO/MOIC/Mos/Block Nodal.
  • Coordination with block officials/NCD Nodal person for issuing the directives/guidance to the ASHA/ANM/CHO/Mos.
  • Ensure the Training need Assessment activity in Block.
  • Support and Coordination in orientation of all Block Nodal persons/officials.
  • Preparation of Detailed block rollout plan for district.
  • Organize and facilitate Training of ASHAs/ASHA Facilitator/ANMs/CHOs/MOs.
  • Ensuring timely execution of training, capacity building activities and follow up.
  • Organize HSC/HWC level ongoing trainings/follow up meetings for Front line health workers.
  • Tracking Progress by using dashboard data and sharing with Block officials
  • Providing inputs on monitoring findings / recommendations to block officials to identify and support low performing FLWS/Villages.
  • Ensuring monthly /Weekly Review and feedback by block officials.
  • Ensuring Training reports /feedback and other documentation.
  • Promotion of developed IEC materials through various forums.
  • Perform as mobile based call center agent to support and resolve the issues facing by ASHA/ANMs/Mos
  • Coordination with other partners working in Block.
  • Travel to various project locations as per need of the project.
  • Any other task assigned by the Team Lead/DPM as and when required.

  • Report to DPM-NCD CoC ICT Interventions
  • Coordinate with other CARE team members/Program Managers.
  • Coordinate with Government counterparts and Partners for Program implementation.

QUALIFICATION & EXPERIENCE
Essential /Preferred

  • Bachelor’s Degree with minimum 2-3years of relevant experience of implementing ICT based, Health and
  • Nutrition projects.
